the file gets dissappeared in FileProvider Extension

I have file provider extension in my Application. In the Files app, the selected file gets disappeared when I tap it(but previewed properly).

If comes back and go the same folder, it loads the disappeared file again. It continues if i choose any file again(disappears).

Do you have any solution to debug or to this issue?
