It can be reproducible by resetting media service through Setting > Developer > Reset Media Service multiple times. Can't be fixed even setting the AVAudioSession inactive when receiving the AVAudioSessionMediaServicesWereLostNotification.

They replied that it will be fixed for a future OS update
