I’m experiencing a similar recurring issue on an Intel MacBook Pro 16-inch (2019, MacBookPro16,1) with a Radeon Pro 5500M 8 GB.
There have been three matching incidents across macOS 26.5.2, 26.6.2 and now 26.7.1 (25G241). During the recent incidents, the Mac was unattended, connected to power, with no external display, dock or wired peripherals.
When I tried to wake the display, a black screen with system text appeared, followed by a restart and WindowServer/macOS problem reports. However, the diagnostics show that the graphics failure began well before my keyboard interaction.
The latest reports show:
20 Radeon GPU-reset attempts, all on channel 18 VMPT, with stalled progress, failed ring write-pointer recovery and GFXOFF errors.
WindowServer’s main thread blocked in IOAcceleratorFamily2.
A userspace watchdog kernel panic after WindowServer failed to check in for 120 seconds, following two induced crashes.
Display state OFF, nominal thermal pressure, and compression/swap reported OK.
My current AC-power settings disable automatic system sleep, so display-off should not be assumed to mean the entire Mac was asleep.
I have sent the native reports to Apple. These findings do not yet distinguish a driver/power-management deadlock from an intermittent hardware fault.
Has anyone with this model or the 5500M found a reproducible fix, or received a confirmed hardware diagnosis, particularly when the issue occurs without an external display? What would you recommend as the next diagnostic step to distinguish a driver/power-management issue from a hardware fault, and when should I seek hardware service?
