happens on my 12, 11, and iPad. Seems to me to occur often after using the iPhone screen record - I had theory that using apple AVExport when app was open had something to do with it, but just can't pin down after 100s of tries. Do you have any other AVExports going on before you see this happen?

eskimo - how do I file a bug? If this is something useful, I'd suggest have Apple Technical Support add this to their process when they tell you that they can't help. I tap on the feedback assistant links and see nothing. Are these what you are referencing?
