I had a similar issue occur where I could start a live activity and my system would also recognize it exists (i.e. it gets dismissed into the dynamic island). What caused it for me was that I had imported a custom styling library dependency to style fonts, colors, etc, but my guess is that the live activity with the custom fonts/styling wasn't accepted by the iOS styling constraints, so it failed silently
