My general advice on this front is that you test things like installer packages in a VM. That allows you to restore the VM to a fresh snapshot, one that’s never seen your product, between each test. If that works, you have a reasonable degree of confidence that your installer package is correct and any problems that show up in the field are with the user’s machine, not your software.

I say “reasonable degree of confidence”, not 100% confidence, because it’s possible that your package has some oddity that’s triggering a problem only on specific machines. If so, you end up playing the statistic game: How often is your package installed, how often does it fail, and are there are common factors in the failures?

But yeah, in this case it sounds like the user’s machine just got itself into a bad way. If you see this crop up again, please grab a sysdiagnose log before restarting. You can then include that in a bug report about this.
