Appears to have been an apple backend (Apple's private server side systems) problem. I reported the problem, to support and got responses asking me for more information (e.g., app name, app id, tester's name, etc.) - provided that information and got several responses such as "we'll investigate further and let you know when we have an update." Finally, one support person forwarded my problem to a "senior Advisor with Development." Once that was forwarded, the problem was resolved in about a day and a half.
