I am having this same problem. To answer your bullets: Yes Yes, there are several accounts on this Mac but only one is affected No No One factor in my case which may be relevant is that I migrated from a previous Mac on to an external drive on the new machine. I had a lot of trouble getting this to work but I got there in the end
