Six years later and still the same problem. The payment form doesn't include a CVV field so no transaction ever succeeds. My application just sits in limbo as Apple haven't received payment. What makes it worse is that Apple removes the callback contact option after you've submitted an application so you can only send emails which takes ~5 days to get a reply.
