Did you get any answer on this? I am going through the same thing. The problem is that LINE 1 on the W9 form on Apple's Agreements page is pre-populated with the LLC's name and doesn't allow modification for the case where an LLC is considered a "disregarded" by the government and therefore the owner MUST put their personal name down on Line 1.
