I tried the form as well, I clicked on submit and nothing happened, then I opened chrome developer tools and inspected the Network Request, turned out there is an API failure going on. I cannot believe Apple has such a low standard when it comes to contacting them on their support! It's been 6 days now and my Developer Program Enrollment is pending:(
