I was able to get it to finally take my payment by selecting "OTHER" rather than allowing automatic city and state. That seemed to help mine go through. But I looked at the response email and it didn't seem to include the State. So if the Apple Developers are looking please FIX THIS for everyone else this took me 2+ months to get this resolved with tons of email, phone call, and still nothing fixed till I used 3 credit cards (no success), and then it was playing with the form input till I got it to work. I just got my confirmation that the payment went through. My credit card company never say anything because the information being sent to the credit card company is WRONG so they auto decline. After doing this fix I was redirected to the "call to confirm" for my credit card company. Time to ship my APP! LONG OVER DUE!
