Unable to access "Re-instate a Terminated Developer Program Membership" form after Developer Program termination

Hello everyone,

I'm hoping someone from Apple Developer Support or anyone who has gone through this process can advise.

My Apple Developer Program membership was recently terminated. The termination notice states that I can contact Apple, and Apple's Legal page also says that terminated developers can petition the App Review Board by completing the "Re-instate a Terminated Developer Program Membership" form.

However, I'm unable to access that form.

Here's what happens:

I open the official Apple Legal page that references the reinstatement form. I click "Re-instate a Terminated Developer Program Membership." I'm asked to sign in with my Apple ID. After signing in, the page never finishes loading. It remains stuck indefinitely. I've tried multiple browsers, private/incognito mode, clearing cookies, and different devices with the same result.

I also contacted Apple Developer Support. They confirmed that the reinstatement request should be submitted through that official process, but I still cannot access the form because of the loading issue.

I'm not asking anyone here to review or comment on my termination decision. I'm simply trying to understand:

Has anyone else experienced this endless loading issue? Is there an alternative way to submit a reinstatement petition to the App Review Board if the official form cannot be accessed? Is there a specific Developer Support team that handles technical issues with the reinstatement form?
