Fixed (as least to my config/situation).
Note: Reverting the workaround:
defaults write com.apple.dt.Xcode IDEDocumentationSpotlightIndexDisable -bool NO

Xcode now behaves as it should. The 'Developer Documentation' window can be called, a re-indexing takes place (indexing takes a few minutes, during this time Xcode consumes a few CPU cycles, but so far everything is acceptable).
The indexing process is completed and a new directory has been created.
/Users/username/Library/Developer/Xcode/DocumentationCache/v279
So far I have not detected any side effects or other errors.
