Same issue here. I have a US single-member LLC owned by a non-US individual, and App Store Connect is only showing the W-9 form. I have contacted Apple Finance Support about this and currently have an open case. I followed up with Finance today, but the W-9 is still showing in App Store Connect. I’ll update this thread if I receive any useful information from Apple.
